"Lina and Scott are going their separate ways," a source told Us. "They ended on a good note as friends, and Lina ended it with him."
News of Disick's first real relationship since Kourtney Kardashian just surfaced last week, so clearly the romance was short-lived.
Disick, 32, just recently got out of rehab, so his struggles with substance abuse and co-parenting three children reportedly contributed to Sandberg's choice to leave him.
"They both have a lot going on," the source added. "He's going to focus on his well-being and being a good dad."
Disick and Sandberg had been spotted out together in Los Angeles in recent weeks. He allegedly thought the model was "really hot" and witnesses saw the pair hugging outside of Beverly Hills Hotel the other day.
"He did care a lot about Lina," the source told the magazine.
Disick and Kardashian, the 36-year-old mother of his children, broke up after nine years together in early July 2015 when he was photographed getting touchy-feely with ex-girlfriend Chloe Bartoli in Monte Carlo.
Disick and Kardashian's kids are Mason, 6, Penelope, 3, and Reign, 12 months. Just last month, reports circulated Kardashian was hooking up with Justin Bieber. The 21-year-old singer told The Bert Show earlier this month he was "being used" by Kardashian.
