Amber Portwood had allegedly threatened to commit suicide on Tuesday, local police have confirmed.

"Late [Tuesday] morning (11:34AM), Anderson Police responded to a call at a residence in the 1300 block of Pleasant Meadow Drive. According to the caller, 21-year-old Amber L. Portwood was depressed and threatening to end her life. The victim, who did not appear to be injured, was transported to a local medical facility for further evaluation," Indiana officials released in a media statement.

According to a police source, the incoming call was made by "a relative" from who was not at the Teen Mom star's home but asked authorities to check on her, Star Magazine reported. Police officials subsequently rushed to Portwood's residence and found her unconscious with a rope around her neck.

"Two policemen arrived, found her face-up on the couch and immediately called for an ambulance. When two EMT workers arrived, they removed the rope that Amber had loosely knotted around her neck," the police source added.

"They tried speaking with her, but she was in and out of consciousness and couldn't respond; she was just moaning. They gave her an oxygen mask, put her on a gurney, and rushed her to the hospital."

Another source told Star the mother of Gary Shirley -- who is the father of Portwood's two-year-old daughter Leah and her on-again, off-again fiance -- was the person who called the police.

MTV has not commented on Portwood's alleged suicide attempt beyond releasing a brief statement to the media regarding the incident on Tuesday.

"Our understanding is that Amber has been admitted to a local hospital, but we have not been able to confirm the accuracy of any reports being circulated and are waiting for further details. Our thoughts are with her and we hope to hear more soon," the network said in a statement.

The Teen Mom star is reportedly still under evaluation at St. Johns Hospital where Shirley recently visited for about five minutes.
