Jenelle Evans has reportedly found herself back in rehab for allegedly abusing heroin.

The 21-year-old Teen Mom 2 star is voluntarily "seeking treatment for her drug addiction issues and related stresses stemming from her tumultuous last few months," a source told RadarOnline.

"Everyone has been worried about Jenelle doing heroin, even though she was denying that for a long time. Jenelle has been through the wringer lately. She got married to a guy that she now thinks is a disaster, got pregnant, had a miscarriage and is basically falling apart."

Evans also filed four assault charges against estranged husband Courtland Rogers and demanded a divorce via Twitter earlier this month. The couple got engaged in November and then wed in early December in a quickie ceremony.

The reality TV star had previously denied using drugs while pregnant with her second child despite Rogers' multiple public accusations. But according to another source, Rogers was correct about Evans' harmful and criminal behavior.

"She has been doing drugs, and she really wants to stop and live a clean life for herself and for her son Jace," the second source told RadarOnline.

Jace has been in the custody of Evans' mother Barbara Evans.

Evans was reportedly committed to a rehab center for 72 hours in November and checked herself into a treatment program in California in 2011 as well.
