The 46-year-old "The Real Housewives of New York City" star took off for the island Monday after helping to fill four planes with food, water and medical supplies for Hurricane Maria victims.

"#workingforyoupuertorico We got a 4th plane headed to PR and working on a 5th!" Frankel wrote on Instagram. "I'm bringing $25k in Costgo gift cards, $25k in cash gift cards and approximately 15k lbs of food, water and medical supplies."

"With the planes return we will be able to transport PR citizens stateside to get medical attention. #Bstrong #100percent #thisisacrisis," she added.

Frankel shared several photos over the weekend of her preparations for the trip. She arranged for planes to take off from New York, Miami, Fla., and Fort Lauderdale, Fla., for Puerto Rico.

"Filling 3 planes with supplies, suitcases full of cash cards, pediatric supplies, insulin, hygiene products, medics, non perishable food, Rescue Warrior nurses, ETMs, combat medics," the star captioned pictures of the efforts Sunday.

"We will return these planes full of PR cancer patients, diabetics and people that are dying," she said. "Please donate to bstrongdelivergood.org to help me with these efforts."

Frankel raised funds through her B Strong program, which supports women in crisis. She partnered with the disaster relief charity Delivering Good "to provide gift cards, bank cards and critical supplies" and help "replace what was lost in the devastating hurricanes."
