Todd Herzog has yet to conquer his life-threatening addiction demons.

The Survivor: China winner shocked viewers with his severe alcohol abuse when he first appeared on Dr. Phil in 2013 completely intoxicated and barely able to walk or talk. After three rehab stints -- two of which were organized by the talk show host -- Herzog has sadly relapsed again.

Dr. Phil recently released a preview clip of the stories that will be featured in November, and one of the segments will be the return of Herzog, seemingly worse than ever before. (Herzog previously sought help on the show in November 2013 and November 2014).

"He has hit rock bottom again," Dr. Phil says in the promo.

"Are you trying to drink yourself to death?" the talk show host asks Herzog, who's sitting in a wheelchair.

"I'm still living, but I don't want to," Herzog replies as he bursts into tears.

Before this episode filmed, Herzog was experiencing seizures, as many as 12 per day. And on the day of his Dr. Phil appearance, the former reality TV star suffered at least three seizures -- one of which occurred right on the set while cameras were rolling.

On November 1, Herzog took to Facebook to explain his grim situation to fans.

"Hello everyone. I wanted to let you know that I have had a relapse recently. An extremely bad one at that. I was fortunate enough for my health, sobriety and life to go back on Dr. Phil and sent to treatment. I Have been here for almost two weeks now and am going to continue fighting my disease of alcoholism for a while," Herzog wrote.

"I'll warn you that the episode of Dr. Phil that will be airing will not be a pleasing one, but if you know me in any way shape or form you know that I'm a fighter, a strong and determined man and I am willing to do anything I have to to kick this demon I have faced in the butt!!!"

Herzog continued in the statement, "Please, if you yourself suffer from addiction or know or have a loved one with addiction help them find the tools needed to live life. I'm not done with my journey of life. Please feel free to send me mail I'll do anything possible I can to write back because I know how alone and scary it can feel."

"I won't be online for a while because I'm focusing on getting myself to 100%," he added. "But rest assured I'm doing fantastic and healthy so far and am not even close to giving up and letting my addiction win! With all the love in the world to each and every one of you."

Herzog's current rehabilitation journey is his fourth attempt at long-term sobriety. He first sought treatment in January 2013, over five years after he competed on Survivor, once his alcohol addiction came to light during a week-long Caribbean cruise.

When he initially appeared on Dr. Phil in late 2013, Herzog admitted he drank an entire bottle of vodka before his public interview. His mother, Shirley Herzog Keeler, also said her openly-gay son would guzzle down up to three bottles of vodka a day and she feared every day he might die.
