Leo earned the honor for her portrayal of a Lowell, Mass., woman who stopped at nothing while managing her sons' boxing careers.
"Mine? For me?" Leo asked presenter Kirk Douglas as she took the stage to claim her statuette. When Douglas, 94, told her she is more beautiful in real life than she was in the film, Leo replied: "You're pretty good looking yourself. What are you doing later on?"
Turning back to the crowd, she exclaimed: "Oh my God! Oh, wow! I know there's been a lot of people who said a lot of real, real nice things to me for several months now, but I'm just shaking in my boots here."
"The Lost Thing" was then named Best Animated Short and "Toy Story 3" declared Best Animated Feature.
Aaron Sorkin earned the accolade for Best Adapted Screenplay for "The Social Network" and David Seidler the trophy for Best Original Screenplay for "The King's Speech."
Earlier in the evening, "Alice in Wonderland" won the Oscar for Best Art Direction and "Inception" picked up the prize for Best Cinematography.
"The King's Speech" led the field going into the evening's competition with 12 Oscar nominations, including one for Best Picture.
The other Best Picture contenders were "Black Swan," "The Fighter," "Inception," "The Kids Are All Right," "127 Hours," "The Social Network," "Toy Story 3," "True Grit" and "Winter's Bone."
Current Best Actor Oscar nominee James Franco and actress Anne Hathaway served as co-hosts of the prize presentation.
